Service Description: | Transition 2 (T2) is a college for young adults with Severe Learning Difficulties and/or Autism, offering bespoke learning pathways based on person-centred long term educational targets which encourage learners to ‘start with the end in sight’.
Transition2 works hard to support young people to develop the skills, knowledge and experience required to establish meaningful and sustainable employment and/or leisure opportunities in their local community, based on their individual interests, assets and qualities.
Working within the context of his/her circle of support, Transition2 facilitates the development of professional and community connections which not allows learners to build important relationships for adulthood but also informs their learning journey and helps them to establish genuine life outcomes beyond college.
Bespoke learning pathways at Transition2 are designed to ‘start with the end in sight’ so from the first term onwards learners are developing the knowledge, experience and skills required to support their transition towards meaningful, sustainable opportunities in adulthood. |
