| Service Description: | We are a hospice for children and young adults aged from birth to 30 years. We provide in-house, residential care as well as community family support and outreach services.
We are able to continue to support many of our young people up to the age of 30 years. We work with adult hospices and the child/young person’s community services to support transition to other services where required. We are a member of the Midlands Transition Taskforce and the Together for Short Lives Transition Pathway Group.
